Signature Page and Volunteer Covenant

 

Each adult member of our co-op must sign and return this signature page to Lighthouse Co-op upon registration. If you have any questions about the statements herein, please ask the director before you sign.


I state that I am not currently, nor have I ever been, involved in a violent crime, sexual misconduct of a criminal nature, or any crime involving a minor.

I understand that I am responsible for the safety of myself and my children at regular co-op meetings, as well as other activities sponsored by Lighthouse Learning Co-op, as set forth in the Family Agreement. I understand that Lighthouse Learning or Hillside Christian Church will  not be held liable for any injuries, illnesses, or other events that may transpire in the normal course of its activities.
